Description

NorthCare is seeking a dedicated Care Navigator to provide exceptional outpatient care navigation services to adults, children, and families in a community-based setting. Key Responsibilities:
  • Coordinate complex cases using evidence-based interventions, ensuring continuity of care and patient empowerment.
  • Collaborate with multi-disciplinary teams and community partners to support health behavior change and treatment goals.
  • Assist patients in accessing essential services such as healthcare benefits, housing, transportation, and educational opportunities.
  • Provide timely crisis intervention and develop safety plans in collaboration with management and therapy teams.

Qualifications

Qualifications:
  • ODMHSAS Certified Behavioral Health Case Manager II (CM II) and Wellness Coach Certification required; CM II/PRSS dual certification preferred.
  • Minimum two years as CM II in community mental health
  • Proficiency in MS Word, strong interpersonal and communication skills, and ability to work independently.
  • Access to reliable transportation, valid Oklahoma driver's license, and automobile liability insurance.
  • Ability to work in various community settings and adapt to changing situations.
  • Access internet hotspot and cellphone for fieldwork
  • Availability for flexible hours, including holidays and weekends as needed.
