We are seeking a dedicated and compassionate Care Coordinator to join our team. In this pivotal role, you will be responsible for managing patient care plans and ensuring seamless communication between patients, families, and healthcare providers. You will assess patient needs, develop individualized care plans, and facilitate access to services and resources. Your organizational skills and empathetic approach will help optimize patient outcomes and enhance their overall experience.

How You Will Contribute

  • General care coordination duties including:
  • Assist clients and individuals and their families with locating plan friendly providers or facilities.
  • Assist in preventing and/or resolving delay in care by educating providers/facilities on insurance.
  • Confirm the deployment and receipt of ancillary services such as home health, DME, radiology, and community based social services.
  • Aid and resolves issues regarding claims, billing, and payment of healthcare services.
  • Educate clients and individuals and their families on benefits plans including items such as: deductibles, in/out of network inquiries, and estimation of benefits (EOBs), and plan types.
  • Provides an excellent experience through direct communication.
  • Calls external facilities and providers to execute on care plan issues for clients and individuals and their families.
  • Make outbound client calls to coordinate individual needs.
  • Assist with locating community resources for clients and individuals and their families.
  • Assist team with faxing, documentation, and telephone support.

Skills & Experience to Be Successful

  • Bachelors degree preferred
  • High school diploma
  • TNLife and Health License (obtain within 90 days of employment)
  • Proficient with Microsoft Office.
  • A minimum of two (2) years of related experience or equivalent, preferably in a physician office or medical provider.
  • Experience in employee benefits industry preferred.
  • Previous experience in medical claims or billing preferred.
