Job Title: Home Health Registered Nurse Position

Job Description:

Responsibilities:

* Provide direct patient care in the home setting, including assessments, treatments, and education
* Develop and implement individualized care plans that address the unique needs of each patient
* Collaborate with other healthcare professionals to ensure seamless care coordination
* Maintain accurate and detailed records of patient interactions and care provided

Requirements:

* Current, valid, and unencumbered Tennessee RN license or Compact license
* Current and valid driver license, transportation, and auto insurance
* One or more years clinical experience
* Self-motivated and customer service oriented
