We are searching for enthusiastic, caring team members for our continuing care community This is a full-time opportunity working 32 hrs/wk generally 8:30am-5pm with schedule flexibility Must have 2+ years of MDS experience and active LPN or RN license in the state of Connecticut. Knowledge of OBRA regulations preferred. Competitive rates based on experiences and credentials.

Skills and Competencies:

  • Ability to read, speak and understand the English language clearly.
  • Strong interpersonal, organizational, and communication skills in person and via telephone.
  • Ability to make decisions and assessments.
  • Strong computer skills; knowledge of MatrixCare and Microsoft Office helpful.
  • Ability to maintain good rapport with staff, families, physicians and residents.
  • Ability to work calmly and efficiently under pressure.
  • Ability to relate to elderly in a dignified manner.
  • Document Medicare Certifications and Recertification's ensuring doctor's signatures are present.
  • Submitting Medicare Denial Letters to Resident/Power Of Attorney (POW)/Health Care Advocate (HCA) per Medicare regulations.
  • Facilitation skills for conducting Care Conferences.
