Responsibilities
- The Patient Coordinator is responsible for enhancing the overall patient experience and ensuring that patients receive the necessary support and resources.
- Accompany patients to their benefit renewal appointments to advocate for the patient and the program.
- Visit patient homes and update patient care.
- Effectively communicate and provide information to patients, caregivers, and physicians.
- Understand HIPAA and maintain confidentiality.
- Display a warm and friendly attitude.
- 1 to 3 years of experience in a Care Team or Patient Navigation Role preferred
- Healthcare experience preferred
- Key competencies include, empathy, problem solving, communication, active listing, and tenacity
- Self-motivated with the ability to manage time appropriately.
- Experience with MS Office products (Outlook, Word, Excel, etc.)
- Minimum associate level education preferred. (High school diploma or equivalent is required.)
- 1 year of medical knowledge preferred.
- Dependable transportation
$19 - $19 an hour
$100 monthly cell phone reimbursement, after 90 days
Monthly in-office lunch to celebrate birthdays, anniversaries, and other events
PTO, paid holidays, paid sick time, after 90 days
Health, Dental, and Vision insurance, after 90 days
Hours typically from 8a-5p
