The Supported Living Supervisor will be report to the Program Coordinator or Associate Director and be responsible for the daily supervision of the Supported Living Program serving individuals with Intellectual/Developmental Disabilities (IDD). In addition to working directly with these individuals, the Supervisor will assist in training and supervising the Direct Support Professionals. The Supervisors training will be hands-on, to provide a model for staff to follow.
**Nature and Scope**:
The Supported Living Program will operate year-round on a seven day a week schedule. The position works with individuals in community-based settings. The Program Coordinator will provide direct care, perform administrative duties, carry a phone and is required to work flexible hours.
**Essential Duties and Responsibilities**:
- Work in the program on a daily basis, providing direct care. Provide daily oversight of home(s), ensuring quality care to our members.
- Work in conjunction with the Program Coordinator on evaluating Direct Support Professionals and keep trainings up to date.
- Work with Direct Support Professionals to develop individualized support plans and training strategies as well as monitoring the progress.
- Work with Direct Support Professionals in implementing Behavior Treatment Plans/proactive strategies to enhance and reinforce member positive behaviors. Update plan with team.
- Oversee medication management.
- Work in harmony with the Program Coordinator/or Associate Director to direct and monitor the work of Direct Support Professionals to ensure that agreements established in the DDD contract are followed and individual files are kept accurate and up to date. (Objectives, skill plans, data sheets, active treatment schedules, quarterly reports, med records, daily staff hour reports, professional contacts, progress notes, incident/seizure reports, face sheets, emergency cards etc.).
- Work in conjunction with the Program Coordinator to train and supervise Direct Support Professionals in assisting adults with eating, personal care, and transferring, demonstrating proper techniques and care. (The position requires lifting and carrying adults, done as two-person transfers.)
- Maintain good working relationships with professionals, staff, families and persons served. Problem solve in a professional, non-hostile manner. Offer options and solutions and give feedback in a constructive manner.
- Attend team meetings and in-service training.
- Promote and follow practices that support health and safety of staff.
- Maintains client confidentiality and protects clients/staff by keeping information secure.
- Provide opportunities for adults to assume and maintain valued roles. Show innovation and insight in developing the community’s ability to provide supports.
- Oversight of Direct Support Professionals on vehicle use. Ensure oil changes, routine maintenance and documentation is maintained and up to date. Submit documentation within established timeframes.
- Carry a cell phone; responsible for answering calls in a timely manner.
- Conform to policies, practices and procedures of Easterseals Blake Foundation.

- Ability to physically lift and complete transfers for members.
- Demonstrate initiative and follow-through in carrying out responsibilities. Demonstrate willingness to accept new responsibilities, accept change and take on responsibility with minimum direction.
**Minimum Requirements**:
- Age: Must be at least 21 years of age.
- 1 year of experience supervising employees, preferred.
- Experience: Any combination of training and experience working with Intellectual/Developmental Disabilities (IDD).
- Must possess a current/valid Arizona Driver’s license and be insurable under the agency’s automobile policy.
- Ability to work flexible schedules: weekends, nights, days, evenings.
- Obtain and maintain current Level One Clearance Card.
- Obtain and maintain current CPR, First, Aid, Article IX.
