**St Ann Health and Rehab** is hiring for a passionate **Social Worker (LSW)** to join our 50-Bed Skilled Nursing facility. As a Social Worker, you will work closely with our residents to ensure quality care is provided to improve the lives of our residents.
Duties and Responsibilities:
- Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
- Identifying individual social and emotional needs
- Developing and maintaining individualized care plans
- Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
- Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
- Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
Qualifications/Requirements:
- Degree in Social Worker
- Licensed Social Worker (LSW) is prefer
- At least one year experience in Social Work in Long Term Care or similar settings
- Experience working with a diverse aging population
