We are seeking a skilled Social Worker to join our team at Advocate Health. As a Social Worker, you will play a critical role in providing patient-centered care and ensuring the well-being of our patients and their families.
Key Responsibilities- Provide psychosocial assessments and develop individualized care plans to address the unique needs of each patient.
- Collaborate with the healthcare team to coordinate care and ensure seamless transitions between departments.
- Connect patients and families with community resources and services to support their health and well-being.
- Document patient interactions and progress in accordance with departmental protocols.
- Participate in quality improvement initiatives to enhance patient outcomes and experience.
- Master's degree in Social Work from a Council on Social Work Education (CSWE) accredited school.
- 1 year of experience in social work, preferably in a healthcare setting.
- IL-LSW Eligible, LCSW preferred.
