- Master's Degree in Social Work: Possess a Master's degree in Social Work from a Council on Social Work Education-accredited program.
- Current Social Worker License: Hold a current Social Worker license in the state of practice, in accordance with state licensure requirements.
- Proficiency in Clinical Skills: Demonstrate proficiency in clinical skills, including assessment, documentation, and patient care planning.
- Excellent Communication Skills: Possess excellent verbal and written communication skills, with the ability to work effectively with diverse stakeholders.
- Advanced Palliative Hospice Social Worker Certification: Hold the APHSW-C certification through the Social Work and Palliative Care Network.
- EMR Experience: Possess experience with Electronic Medical Records (EMRs).
