**Case Coordinator**

Rehabilitation Support Services, Inc. is hiring a Case Coordinator for its Apartment Program. **Hours and Benefits**:

- 40 hour work week, Monday through Friday
- Eligible for Overtime Pay and Holiday Double-Time Pay

**Requirements for the Case Coordinator**:

- A high school diploma or equivalent
- Valid NYS Driver’s License required for most positions.
- Successful clearance by the Justice Center and New York State Office of Children and Family Services Statewide Central Register Database Check also required.

**Responsibilities for the Case Coordinator include**:

- Responsible for a designated case load; including the accurate and timely completion of case record documentation in the Agency electronic health record.
- Establish and maintain good rapport and therapeutic relationship with residents.
- Aid and encourage clients to meet the established goals of their service plans.
- Teach, monitor and evaluate client’s skills of daily living.

**Job Types**: Full-time, Part-time

Pay: $16.00 - $18.40 per hour
