**Intake specialist Job Requirements**

**Intake Specialist Job Summary**

The Intake Specialist position requires organization, attention to detail, and an aptitude for creating and following schedules. Require administrative tasks such as placing phone calls and conferring with the clients. The main purpose is to coordinate events, and services.

**Intake Specialist Duties and Responsibilities**
- Communicate between staff, patients and family.
- Handle patient case management.
- Scheduling and creating detailed calendars
- Develop patient goals and monitor progress.
- Maintain patient confidentiality and quality care.
- Lend support and/or independently prepare various reports.
- Contacts agencies/hospitals/companies/personnel as needed and/or as appropriate.
- Explain the function of home health team members who will implement the plan of treatment to the patient.

- Ensure effective communication and collaboration with branch staff and field sales resources.
- Inform hospital personnel, patient and/or patient’s families of case acceptance and document all demographic, clinical, payer and other information as required on company approved forms to support diagnosis and home healthcare orders.
- Provide input to care plan development and inform the company location clinical staff.
- Maintaining relationship with office staff, clients, and caregivers
- Excellent analytical and time-management skills
- Proficient computer skills, including Microsoft Office
- Able to follow multiple guidelines and procedures

**Job Type**
- Full Time

Intake Specialist earns an average. Salary may depend on the level of education and experience.

**Experience**

**-**Homecare: 1 or 2 years (preferred but willing to train)

**Schedule**
- Monday - Friday 9:00am-5:00pm

Pay: $22.00 - $25.00 per hour

Expected hours: 40 per week
