**Hospice Admissions Nurse**

**The Admissions Nurse** is an experienced registered professional nurse and an Interdisciplinary Team member whose primary responsibility is to present Heartford Hospice services and philosophy to patients and families in a manner that results in the admission of appropriate patients.
**Essential Job Responsibilities**:
- Participation in the information gathering, analysis, education, and form completion aspects of the admissions process
- Review clinical documentation for hospice appropriateness and accuracy
- Obtain agreement for admission of referred patients and families who meet admission criteria.
- Build effective referral source relationships to support the high quality and responsiveness expectations associated with HEARTFORD HOSPICE.
- Maintain clinical and professional competence.
- Take the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons.
- Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient’s family has concerning hospice and palliative care.
- Completes all documentation within 24 hours from time of evaluation/ admission and upload all relevant paperwork into the system.
- Assist with developing training material and in services for communities and patient families during downtime.
- Assists with QAPI initiatives and other job duties as needed.
**Position Qualifications**
- Registered Nurse with valid CA state license.
- One year of recent hospice experience preferred with a strong foundation of acute care experience.
- Maintains a current CPR certification.
- Knowledge of the principles of the intake, admission, and transition to care process with the ability to further develop skills in these competency areas.
- Ability to communicate effectively with patients and families, physicians, referral sources and colleagues.
- Knowledge of clinical manifestations of diseases and ability to develop HEARTFORD HOSPICE specific clinical knowledge.
Pay: $52.00 - $58.00 per hour
Expected hours: 15 - 20 per week
