California Hospice Services, Inc **is growing and looking for an experienced, compassionate Community Liaison to join our team** As a Community Liaison with California Hospice Services, you will have the opportunity to advance your career while receiving strong compensation and excellent benefits.

**Responsibilities**:
1. **Developing and Maintaining Relationships**:

- Cultivate and nurture relationships with healthcare professionals, including physicians, social workers, discharge planners, and other key decision-makers in hospitals, nursing homes, assisted living facilities, and other healthcare settings.
- Collaborate with healthcare providers to identify patients who could benefit from hospice care and facilitate timely referrals.

2. **Marketing and Outreach**:

- Develop and implement marketing strategies to promote hospice services within the community.
- Organize and participate in community events, health fairs, and educational seminars to raise awareness about hospice care.
- Create marketing materials such as brochures, flyers, and presentations to effectively communicate the benefits of hospice services.

3. **Assessment and Evaluation**:

- Conduct initial assessments of patients referred to hospice to determine eligibility and appropriateness for hospice care.
- Work closely with the hospice interdisciplinary team to ensure a comprehensive and individualized care plan for each patient.

4. **Coordination and Communication**:

- Serve as the primary point of contact for healthcare professionals, patients, and families seeking information about hospice services.
- Coordinate the admission process for patients entering hospice care, including coordinating with healthcare providers, scheduling assessments, and ensuring all necessary documentation is completed accurately and in a timely manner.

5. **Documentation and Reporting**:

- Maintain accurate and up-to-date records of all interactions with referral sources, patients, and families.
- Prepare regular reports on referral activity, market trends, and competitor analysis to inform strategic decision-making.

**Qualifications**:

- Bachelor's degree in marketing, healthcare administration, business, or related field preferred.
- Previous of 5+ years experience in healthcare sales, marketing, or related field preferred.
- Strong interpersonal and communication skills with the ability to build rapport and establish trust with diverse stakeholders.
- Knowledge of hospice regulations, reimbursement policies, and healthcare industry trends.
- Ability to work independently, manage multiple priorities, and meet deadlines in a fast-paced environment.
- Proficiency in Microsoft Office Suite and CRM software.

Pay: $75,000.00 - $120,000.00 per year
