- **Primary Objectives**:

- The Care Coordinator is responsible for using a collaborative and coordinated approach to linking participants with appropriate health and social services to address their specific needs and treatment goals. This role requires that the Care Coordinator is knowledgeable of relevant community resources and effective at communicating with other professionals, providers, and participants to successfully link participants to services. The role of the Care Coordinator is to offers support services to participants to increase self-efficacy, self-advocacy, basic life skills, coping strategies, self-management of biopsychosocial needs, benefits and resources, and reintegration into the community. This is a full-time position that is 100% devoted to case management for grant participants.**Responsibilities**:

- Registration or Certification with a state approved AOD certifying body. Must be registered or certified upon hire.
- In collaboration with the multi-disciplinary treatment team, the Care Coordinator will assess participants to address any problems, needs, or barriers identified in the areas of: Housing, Employment, Education/Training, Financial, Legal, Sobriety/Social Support, SUD Treatment, Family/Social, and Medical/Psychiatric.
- Assists the primary counselor with discharge-planning efforts, commencing at the time of admission on all assigned participants, including housing, employment, medical/psychiatric, financial, aftercare, etc.
- Provides participants with high-quality referrals and linkages for patients to necessary resources and services as identified in the Treatment Plan/Problem List, playing an active role in reducing access barriers and ensuring participants have ‘actual’ access to needed services.
- Acts as a bridge between health and human service providers to ensure that information is appropriately exchanged, and participants are successfully linked to needed resources/services.
- Assists participants with transitions within the SUD system of care, referring to higher or lower level of care as appropriate.
- Follows up with participants to ensure all scheduled appointments and linkages are completed.
- Collaborates and communicates fully with other team members regarding participant behavior and provides weekly updates of participant’s progress.
