**Responsibilities**:
The High Risk RN Coordinator serves as an advocate and assists patients to reach optimal function and health in an appropriate environment that supports their needs. This position provides coordination across the continuum of care, from inpatient to outpatient to community. The High Risk RN Coordinator applies the process of assessment, care planning, implementation, monitoring, evaluation and coordination of care to meet the patient’s health care needs across the continuum in collaboration with Primary Care Provider, interdisciplinary care team members, payers, and social service organizations to ensure safe, quality and cost effective care. This position works closely with individuals to address health disparities, provide health education, ensure closed-loop referrals, and connect people to resources that can improve their overall health and well-being. The High Risk RN Coordinator will serve the complex needs of our highest risk patient population by meeting them in person, via videoconferencing or over the phone.

Qualifications:
**Required**:

- Current CA Registered Nurse License
- One (1) year previous inpatient/outpatient medical case management experience
- Three (3) years direct patient care nursing experience
- One (1) year of community/population health or related experience
- Current CA Driver License
- Current and ongoing Motor Vehicle Report (MVR) that is considered insurable under the CMHS insurance coverages
- Proficient in Microsoft Office Applications

**Preferred**:

- Bachelor’s or Master’s Degree in a related field
- Bilingual in English/Spanish
- Practice experience in evaluation and management of complex medical and psychosocial issues
- Demonstrated skills in clinical quality improvement and in resource management
- Previous Project Management and Quality Improvement experience
- Strong analytical/problem-solving, written and verbal communication skills
