**Job Summary and Qualifications**:
The Social Worker (BSW) is responsible for promoting patient-centered care by evaluating psychosocial needs and providing community service coordination for identified patient populations. As a member of a multidisciplinary health team, the BSW acts as a liaison between the patient and community resources/post-acute care providers. The BSW creates and facilitates the timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The BSW facilitates patient movement across the continuum of care. The scope of this role includes primarily Med/Surg units.

What you will do in this role:

- 1. Acts as a member of the interdisciplinary team (IDT).

2. Collects information from the medical record, physician, nursing, care coordinator, other members of the IDT, patient, and family to identify financial, social, medical, and follow-up needs post discharge.

3. Collaborates with the members of the interdisciplinary team to include the medical staff, patient, and family to develop a safe discharge plan. Reviews the medical record and meets with the team on a regular basis to identify changes to plan.

4. Educates patients/families regarding advance medical directives and assist with completion when needed.

5. Educates patients and families to the patient’s needs post discharge and informs them of the alternatives available to meet those needs.

6. Refers patients who require extended care post discharge to LTACH, skilled nursing, and long term care facilities. Once an appropriate bed is located, arranges for the patient’s transfer.

7. Collaborates with community agencies to provide services to assist with housing, financial, transportation, educational, home health, and home medical equipment needs of patients/families post discharge. Arranges these services when needed.

8. Documents discharge planning efforts in the patient’s medical record congruent with departmental policies.

9. Provides ongoing education for the other members of the health care team related to ethical, psychosocial and discharge planning issues.

10. Identifies and reports occurrences of physical, emotional or sexual abuse and/or neglect.

11. Serves as a resource contact and information source to patients, families, significant others, and staff.

12. Actively supports and participates in the development and future changes of the department, as well as, orientation and education of new staff members.

13. Other duties as assigned.
