**Job Details**: Family Services of Westchester is looking for full-time Community Wellness Specialist in Purchase, NY.

**POSITION TITLE**: Community Wellness Specialist

**SALARY**: 26$/hour

**STATUS**: Full-time

**ESSENTIAL DUTIES**:

- Conducts both in-person and digital/remote outreach (phone, text and virtual)
- Attends/tabling at community-based events such as heritage festivals, walks/runs and local events
- Conducts screenings utilizing the Mental Health Wellness & Social Needs Screening Tool
- Facilitates referrals and linkage to community resources
- Provides basic health education and interactive educational activities

**MINIMUM REQUIREMENTS**:
Education: Bachelor's degree
