**Social Worker - Outpatient Primary Care Charles B. Rangel Community Health Center Ambulatory Care Network Columbia**

Join our Amazing Social Worker team in our Primary Care setting with Internal Medicine and OB/GYN patients. You will work collaboratively with providers to help support patients and families to obtain the optimum level of health.
You will conduct psychosocial assessments, link patients with community resources, and attend interdisciplinary rounds.
Attend monthly social work meetings and other clinical / administrative meetings connected to the role.
This is a Monday-Friday day shift position
No weekends no holidays
**Preferred Criteria**:
- Bilingual Spanish
- Community experience
- LCSW
**Required Criteria**:
- Master’s Degree in Social Work (MSW)
- New York State License as a Licensed Masters Social Worker (LMSW)

**Salary Range**:
$39.41-$52.77/Hourly
