Job Summary:

The Social Worker is responsible for providing support, resources, and advocacy to individuals and families in need. This role involves assessing clients' needs, developing intervention plans, and connecting clients with appropriate services to improve their quality of life. The Social Worker will work collaboratively with other healthcare professionals, community organizations, and government agencies to address social, emotional, and economic issues that affect clients.

Key Responsibilities:

  1. Client Assessment and Evaluation:

    • Conduct comprehensive assessments of clients' needs, strengths, and challenges.
    • Develop individualized service plans based on assessment findings.

  2. Case Management:

    • Coordinate and monitor services provided to clients, ensuring they receive appropriate care and support.
    • Maintain regular contact with clients to evaluate progress and adjust service plans as needed.

  3. Crisis Intervention:

    • Provide immediate support and intervention during crisis situations.
    • Develop safety plans and connect clients with emergency resources.

  4. Advocacy and Resource Connection:

    • Advocate for clients' rights and access to services.
    • Assist clients in navigating social service systems and accessing resources such as housing, healthcare, financial assistance, and education.

  5. Counseling and Support:

    • Provide individual, family, and group counseling to help clients cope with challenges.
    • Offer emotional support and guidance to clients dealing with trauma, loss, and other life changes.

  6. Community Outreach and Education:

    • Conduct outreach activities to identify and engage underserved populations.
    • Develop and deliver educational programs and workshops on relevant social issues.

  7. Collaboration and Coordination:

    • Work closely with healthcare providers, schools, law enforcement, and other community organizations to coordinate care and services.
    • Participate in multidisciplinary team meetings to discuss client cases and develop comprehensive care plans.

  8. Documentation and Reporting:

    • Maintain accurate and confidential client records, documenting assessments, service plans, and progress notes.
    • Prepare reports for supervisors, funding agencies, and other stakeholders as required.

  9. Professional Development:

    • Stay informed about current social work practices, policies, and regulations.
    • Participate in continuing education and professional development opportunities.

Qualifications:

  • Education: Bachelor’s degree in Social Work (BSW) required; Master’s degree in Social Work (MSW) preferred.
  • Licensure: Current state licensure as a Social Worker (LSW or LCSW) required.
  • Experience: Minimum of 2 years of experience in social work or related field, with experience in case management and counseling preferred.
