**What you will do**:
Supervise, and oversee Inpatient Acute Rehabilitation and Sub-Acute Rehabilitation Admission RN facilitators.

**Responsibilities**:

- Ensure the accurate, efficient, and timely screening and admissions for all patients accessing services at these post-acute care sites of St. Peter’s Health Partners (SPHP).
- Manage and assist in providing information to the general public/referral sources to facilitate access to SPHP’s IRF and SNF subacute programs.

**What you will need**:

- Current NYS Registered Professional Nurse licensure
- 7 years of progressive clinical experience in intake, assessment and admissions; preferably in a hospital and LTC environment.
- Demonstrated supervisory experience.
- Must demonstrate ability to manage staff and build/foster relationships with outside entities
- Must demonstrate knowledge of medical terminology and insurance coverage.
- Excellent customer service skills.
- Valid driver’s license and vehicle for ability to travel between multiple sites.

Pay Range:$35.80 - $47.28

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
