Schenectady Center is seeking a skilled RN Admissions Nurse to join our team in Schenectady, NY.

Job Summary

We are looking for a compassionate and detail-oriented RN Admissions Nurse to initiate admission assessments, develop care plans, and coordinate with the interdisciplinary team. The ideal candidate will have excellent clinical assessment skills, strong leadership abilities, and proficiency in MS Office.

Responsibilities
  • Initiate admission assessments and develop initial care plans for new residents and hospital returns
  • Assist in the development and direction of the Nursing Department as directed by the DON
  • Review PRI's and coordinate all components of the admission process with the interdisciplinary team
  • Audit records to ensure compliance with facility standards
  • Participate in discharge planning as necessary
  • Review and evaluate residents' care plans when conditions warrant a change
  • Perform required charting according to policy and procedure
  • Complete MDS, care plans, and other documents as assigned
  • Maintain confidentiality of all resident information as per HIPAA and Corporate guidelines
  • May be assigned to supervise or direct resident care as necessary
Requirements
  • Active and unrestricted NYS Registered Nurse License
  • Prior experience as an admissions nurse in LTC settings
  • Excellent clinical assessment skills
  • Ability to make good independent decisions
  • Strong leadership and supervisory skills
  • Computer savvy; proficient in MS Office (Excel, Word, Outlook)
  • Long-Term Care Experience Preferred
  • Excellent communication and interpersonal skills
