Hospice Rn Weekend Triage

We are currently looking for RN's to join our team every weekend from **Friday at 5 pm to Monday at 8 am** The Triage Nurse is responsible for providing hospice care for patients and their families during their working hours and responding to phone calls for after-hours needs.
- Triage nurses manage and respond to nurse-directed telephone calls from patients, families, and communities
- **Responsible for patient needs across the entire patient census including West, Central, and East valley**:
- Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act
- Must evaluate and assess the patient's condition based on their signs and symptoms utilizing established triage protocols and guidelines
- Uses health assessment data to determine nursing diagnosis
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
- Obtains appropriate input and direction from the provider as needed to determine ongoing plan of care and recommendations for the patient
- Counsels the patients and the family/caregiver in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan
- Accurately documents symptoms, nursing assessment, education to caregiver/POA, and patient/caller response. Follows policies, procedures, and protocol
- May be required to complete admissions as needed to support the needs of the company
- Communication
- Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given by end of shift. Records pain/symptom management changes/outcomes as appropriate.
- Communicates with triage team and physician using company approved methods (ex. secure messaging system-Teams, phone call)
- Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
- Communicates with community health related persons to coordinate the care plan.
- Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group.
- Provides and maintains a safe environment for the patient.
Pay: $88,000.00 - $90,000.00 per year
