Key Responsibilities:
- Comprehensive Care Planning: Our Hospice RN Case Managers are responsible for planning, organizing, and directing hospice care through the nursing process, which includes assessment, planning, interventions, implementation, and evaluation.

Qualifications:
Minimum one (1) year of experience as an RN in a medical-surgical/acute care setting. Hospice experience is a plus. Proficiency in computer skills, including typing and familiarity with various programs, is required, along with a background in EMR.
