**Position Summary and Job Duties**:
**Provide support and advocacy for clients who are seeking to overcome financial, personal health or family problems. Conduct informal individual and family assessments to assist clients in determining what services are needed. Provide linkage to resources and service providers. Work closely with neighborhood groups and stakeholders to leverage resources and services. Develop a network of providers and stakeholders to promote safety, permanency and well-being for children, youth and families. The goal of this position is to stabilize children, youth and families and avoid their entry into emergency systems of care. This position will serve a targeted neighborhoods and will aim to strengthen families as well as the community. Supports and promotes the Mission, Vision and Guiding Principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and "Commitment to My Co-Workers."**

**40%- Supportive Services**
1) Provide prevention/early intervention services to children, youth and families.
2) Engage clients through a highly accessible, responsive and empowering approach.
3) Conduct assessments through interviews, home visits, and collateral contacts in order to determine the range of options available to clients.
4) Partner with clients in order to build on strengths, enhance protective factors, and decrease risks.
5) Support clients in navigating systems of care in order to get their identified needs met.
6) Directly connect children, youth and families with resources and service providers.
7) Provide consultation to county workers and other regarding community resources.
8) Act as a community referral source and resource person.

**25%- Relationship Management**
1) Build and maintain a process with community partners for service requests and referrals.
2) Engage in collaborative activities with neighborhood groups and other stakeholders.

**Job Requirements, Education, Training & Experience**:

- Graduation with a bachelors degree from an accredited 4 year college or university with major course work in social work, criminal justice or related field.
- One year experience working in child welfare, youth justice, or related field.
- Must possess and maintain a current unrestricted (not due to corrective lenses) driver's license and auto insurance pursuant to the Rock County Administrative Policy and Procedure Policy 5.02 and 5.27
