**Position Summary and Job Duties**:
**This position is a member of a multi-disciplinary team serving individuals with serious and persistent mental illness and/or co-occurring substance use disorders. The position provides virtual, office, and community based case management services intended to maintain functioning and facilitate recovery. Supports and promotes the Mission, Vision, and Guiding Principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and "Commitment to My Co-Workers."**

**50% Provide Direct Services**
- Manage a caseload of individuals who are receiving prescriber services with Rock County Counseling Center.
- Provides various assessments- Participates with recovery teams as assigned-
- Engage in check ins and reminder calls as indicated to monitor stability and increase prescribe appointment attendance, including providing or arranging transportation, follow up calls, and letters if appointments are missed-
- Provides medication monitoring support-
- Provides advocacy for identified clients-
- Provides linkage and support for service referrals-
- Provides crisis intervention as needed-
- Attend prescriber appointments when indicated**25% Provide Service Facilitation functions**- Facilitates the assessment process-
- Collaborate with natural supports, internal and external partners-
- Facilitates the development of the service plan as well as ongoing monitoring and review**20% - Miscellaneous duties**
- Ensures compliance of all programs and medical records
- Attends weekly staffing meetings with each prescriber as needed
- Attends all program/department meetings as required
- Complete 100% Staff Time Entry

**5% - Performs other duties as assigned**

**Success Factors (KSA's)**:

- Professional expertise in the following areas preferred. Training and ongoing professional development will be provided.
- Knowledge of laws, regulations, and practices pertaining to federal and state human service programs
- Knowledge of assessment and treatment of mental illness and substance use disorders
- Knowledge of psychotropic medications, their main effects, side effects, and abuse potential
- Knowledge of psycho-social rehabilitation services
- Knowledge of State and Federal entitlements
- Ability to maintain accurate records and prepare clear and concise reports
- Ability to establish and maintain effectively working relationships within the agency and community
- Ability to understand and carry out verbal and written instructions
- Ability to follow all departmental as well as divisional policies and procedures

**Job Requirements, Education, Training & Experience**:

- Graduation from an accredited college or university with a degree in social work or related field. Experience working in the mental health field preferred.
- Valid Drivers License, Insured, Own vehicle available
- Must possess and maintain a current unrestricted (not due to corrective lenses) driver's license and auto insurance pursuant to the Rock County Administrative Policy and Procedure 5.02 and 5.27.

**Essential Job Functions (physical elements, equipment and working conditions)**:
**Physical elements**:

- Ability to travel throughout the community.
- Potential for interactions with animals including but not limited to household pets.
- Must be willing to transport clients
- Potential exposure to challenging and unpredictable environments.
- Participate in disaster evacuations and fire drills when circumstances warrant. Maintain safety procedures.
- Lifting and carrying up to 25lbs on occasion.

**Equipment use**:

- Ability to use a computer and keyboard.
- Must become efficient at using a computerized documentation within 2 months of hire including the use of Microsoft Office products, an electronic medical record system, and other web based systems.
- Ability to use a copy and fax machine as well as a cellphone.

**Working conditions**:

- Able to work in multiple locations including the office, client's homes, and a variety of community locations.
- Available to work a flexible schedule including evenings when needed to coordinate client emergencies, care, and recovery plan services.
- Potential exposure to physically or verbally abusive persons.
