The County of Riverside's Public Health department has an opening for Social Services Practitioner III with the Maternal Child Adolescent Health. The program seeks to improve the health and well-being of women, infants, children, and adolescents throughout Riverside County utilizing evidenced based knowledge from nursing, social, and public health science. With an equity lens, we partner with the community to achieve positive health outcomes for individuals, and families throughout Riverside County. The Social Services Practitioner III position will work with pregnant women up to six months post-partum that identify as African American. Additionally, this position will conduct client enrollment activities, which includes orientation, informed consent, and the initial assessment, refers participant to FHAs for on-going case management, conduct case conferencing with local BIH staff and any other members of a multidisciplinary team, develop and maintain relationships with local mental health professionals for client referrals, participate in the group sessions that focus on mental health issues by being available to answer participant questions and provide support to the group facilitators, provides mental health education to clients when requested by the FHAs, and conduct trainings on the basics of maternal and infant mental health. Lastly this position will require some field work to include home visitations therefore a California drivers license is required.
This position will work a 9/80 schedule.
Under general direction, performs difficult social service casework dealing with complex individual and family problems or social service casework for patients and their families in healthcare facilities; plans, organizes, implements, and evaluates all clinical services provided to pediatric patients and needs of children in required assignments; may participate in and assist with conducting in-service training programs; may provide leadership as a program consultant in the utilization of casework concepts and methodology; performs other related duties as required.
The Social Services Practitioner Ill is a broad range journey to advanced level classification of the professionally trained social caseworker in the Social Services Practitioner series. Incumbents are characterized by the responsibility for performing full scope social service casework often requiring difficult diagnosis and intensive treatment services. The Social Services Practitioner Ill is distinguished from the Social Services Supervisor series by the latter's responsibility to provide full time supervision of staff performing casework.
