Job Summary:

The Medical Social Worker III provides comprehensive assessments, coordination, and implementation/evaluation of professional social services to patients and their families in order to aid them in coping with social, emotional, and economic difficulties related to medical problems or which predispose to illness.

Key Responsibilities:
  • Uphold Kaiser Permanente's Policies and Procedures, Principles of Responsibilities, and applicable state, federal, and local laws.
  • Provide quality services by assessing patient/family system needs, developing and implementing treatment plans in accordance with departmental and medical center policies.
  • Perform bio-psychosocial assessments, provide crisis intervention/treatment, and offer counseling services to adults, children, and families in emergent situations.
  • Develop culturally sensitive assessments, provide professional interpretations of patients' conditions, and recommend appropriate psycho-social interventions and/or treatment plans.
  • Provide patient education on subjects related to psycho-social adjustments to medical illness, individually, in classes, or groups.
  • Take responsibility for implementation of social work knowledge and values, standards of practice, and enhancement of professional skills.
  • Assess patients' social, emotional, and behavioral problems/elements as they relate to their medical condition and treatment.
  • Utilize professional knowledge in facilitating staff and other meetings, and demonstrate knowledge of and observe department policies and procedures.
  • Participate in Quality Management activities based on member needs, organizational goals, and professional standards.
Interdisciplinary Functions:
  • Function as an effective team member, consultant, and liaison to medical, nursing, ancillary staff, and to community agencies.
  • Demonstrate an interdisciplinary approach, competent follow-up, and communication of accurate information to team members.
  • Assume responsibility for patient, staff, and system problems as indicated by requests for involvement.
  • Demonstrate awareness of the Departments mission, scope, and vision in the context of the Medical Centers structure.
Professional Conduct:
  • Demonstrate professional behavior and interventions consistent with departmental policies and procedures, JCAHO, Title 22, and NCQA legal mandates, professional standards, NASW, ABECSW, and other Medical Center Guidelines.
  • Utilize supervision and consultation appropriately as indicated by regular attendance at supervisory/consultation sessions, identifying and discussing problem cases/situations with supervisor, and seeking consultation with peers.
  • Contribute and share community resources with colleagues.
  • Represent the Department in appropriate medical center and/or community committees.
  • Identify improvement opportunities for the department or work area and communicate ideas to supervisor.
  • Participate in Quality Management activities based on member needs, organizational goals, and professional standards.
  • Identify and participate in the solution to system problems.
  • Attend and occasionally facilitate staff/education meetings.
  • Avoid unscheduled absences, arrive at work at scheduled times, and take action to avoid recurring absences.
  • Plan and submit elected time off in advance.
  • Exhibit standards of professional behavior in compliance with Regional Quality of Service Guidelines.
  • Maintain confidentiality regarding patient and/or family issues in accordance with the Medical Center policy and professional standards.
  • Demonstrate knowledge of and comply with Fire & Safety, Disaster, Universal Precautions, and Infectious Disease policy and procedures.
  • Work in a self-directed manner as demonstrated by requesting assistance when clinical case or situation is beyond experience or abilities.
Productivity:
  • Maintain patient access and meet established standards for effectiveness and efficiency.
  • Prioritize, plan, and organize workload needs within assigned hours.
  • Case find and screen high-risk patients according to Department protocol.
  • Spends at least 70% of time in direct service activity.
  • Respond to referrals with departmentally delineated time frames.
  • Complete and submit statistics in accordance with Departmental policies and procedures.
  • Document interventions in appropriate formats, legibly, and within department time frames.
