**Main Purpose of Job**:
This Community Health Nurse II position will serve as the HIV Service Coordinator. The position will provide client advocacy/case management services to eligible clients. The position will also provide linkage to resources in the community, working with other Health Departments, Social Services, Primary care providers, insurance companies and the Caroline County Department of Corrections and any other agency or resource provider.
**MINIMUM QUALIFICATIONS**:
**Education**: A Bachelor’s degree in nursing from an accredited college or university.
**Experience**: None.

**LICENSES, REGISTRATIONS AND CERTIFICATIONS**:
2. Employees in this classification may be assigned duties which require the operation of a motor vehicle. Employees assigned such duties will be required to possess a motor vehicle operator’s license valid in the State of Maryland.
**SPECIAL REQUIREMENTS**:
Employees in this classification will be required to provide the facility with a telephone number at which they can be reached.
