Pediatric Patient Referral Specialist and Marketer

We are looking for a dynamic and dedicated Patient Referral Specialist and Marketer to join our team.

**Job Description**:
The Patient Referral Specialist and Marketer will play a crucial role in expanding our pediatric and young adult clients base and ensuring seamless transitions for new patients into our care. This role involves both marketing activities to promote our services and the management of patient referrals to ensure a smooth onboarding process. This role is geared toward Pediatric patients and Medicaid Recipients.

Key Responsibilities_**

**_
Marketing and Community Outreach:_**
- Assist with the development and implementation of marketing and sales strategies to promote Northbay Home Health services with knowledge of pediatric clients and Medicaid recipients.
- Establish and maintain relationships with healthcare providers, physicians, hospitals, clinics, and other referral sources.
- Organize and participate in community events, health fairs, and other marketing activities as needed.
- Create marketing materials such as brochures, flyers, and social media content.

**_ Patient Referral Management_**
- Receive and process patient referrals from various sources.
- Clinical background, payer knowledge especially within public assistance is vital, also problem-solving skills, you’ll work with physicians, discharge planners and patient's families to plan the best continuum of care.
- Communicate with referral sources to gather necessary patient information, clinical diagnosis and ensure a smooth referral process.
- Coordinate with the clinical team to schedule initial patient assessments (aka meet-and-greet) and services.
- Performs clinical assessments of referred clients, then counsels with client's family members to assist them in making the best care decisions
- Partners with hospital case managers to create smooth patient transfer processes and help remove payer-related and operational barriers to getting appropriate patients into this highly effective level of care.
- Maintain accurate records of referrals and follow up with referral sources as needed.

**_ Customer Service_**
- Serve as the primary point of contact for referral sources and new clients, remaining professional to ensure that quality of standard we have here at NBHH
- Provide excellent customer service and address any concerns or questions from clients and family members and referral sources.
- Ensure overall client satisfaction by facilitating smooth transitions into home health care services.

**_ Collaboration_**
- Work closely with the clinical team to understand patient needs and coordinate care.
- Collaborate with the management team to develop and refine marketing strategies.
- Provide feedback and insights from referral sources to improve service delivery and patient care.

**_ Requirements_**
- Associate of Art’s in Science, Bachelor's degree in Marketing, Healthcare Administration or Business Marketing Development in Healthcare or a related field.
- Minimum of 2 years of experience in healthcare marketing or patient referral management.
- Strong understanding of home health care services and the healthcare industry.
- Excellent communication and interpersonal skills.
- Ability to build and maintain relationships with referral sources and community partners.
- Strong organizational skills and attention to detail.
- Proficiency in Microsoft Office Suite, various EMRs and CRM software.
