**Job Summary**:
As an LNA at NCHHHA, you will have the ability to get to know your individual patients and support your team members in executing tailored plans of care for Home Health and Hospice.

The Facility Based Hospice Aide position is a highly rewarding position that helps clients who are receiving hospice care in a facility setting by improving their quality of life and contributing to a safer environment. Hospice Aide assists individuals with personal care, activities, aroma therapy, as well as emotional support. He/she administers personal care and assistance with activities of daily living for patients in the facility setting under physician’s orders, assignment and supervision provided by the Clinical Nurse Manager. Facility Based Hospice Aide must be highly self-motivated, non-judgmental, flexible, possess excellent communication skills, and be able to work independently. Preservation of confidentiality is mandatory.

**Essential Qualifications**:

- Must have current active NH LNA License
- Ability to demonstrate flexibility in response to unexpected changes in work volume and work schedule, required
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability or financial condition of the patient.
- Must possess the disposition and personality to work harmoniously with a wide variety of people and all levels of staff and interact professionally as a member of a multi-disciplinary team.
- Speak, write, read and comprehend English.
- Computer proficiency in electronic medical records data entry.
- Maintains valid driver’s license and certificate of auto insurance coverage.
- Current BLS Certification required.
- Access to operable, insured motor and valid driver’s license, good driving record and maintain personal auto liability coverage, as specified in NEAH policy
