As a Licensed Nursing Assistant (LNA) at North Country Home Health & Hospice Agency, you will have the opportunity to build meaningful relationships with patients while collaborating with a dedicated team to implement personalized care plans for Home Health and Hospice.
The Facility-Based Hospice Aide role is profoundly fulfilling, assisting clients in a facility environment who are receiving hospice care. Your contributions will enhance their quality of life and foster a safer atmosphere.
In this position, you will provide essential personal care, engage in therapeutic activities, and offer emotional support to individuals in need. You will administer personal care and assist with daily living activities for patients in a facility setting, following physician directives and under the guidance of the Clinical Nurse Manager.
Successful candidates will demonstrate self-motivation, a non-judgmental attitude, flexibility, and excellent communication skills, with the ability to work independently while maintaining confidentiality.
Key Qualifications:
- Current active NH LNA License
- Ability to adapt to unexpected changes in workload and schedule
- Capacity to work effectively in diverse home situations, regardless of patient background
- Strong interpersonal skills to collaborate with a variety of individuals and team members
- Proficient in English communication, both written and verbal
- Experience with electronic medical records data entry
- Valid driver's license and auto insurance coverage
- Current BLS Certification
- Access to a reliable vehicle with a good driving record
- Commitment to obtaining hospice and palliative care certification within one year of hire
