Littleton, United States North Country Healthcare Home Health & Hospice Agency - OCC THERAPY Full time

Provides services to clients who haveimpairments, functional limitations, disabilities, or changes in physicalfunction and health status resulting from injury, disease or other causes. Provides prevention and wellness servicesincluding screening and health promotion. Interacts and practices in collaboration with a variety ofprofessionals.

QUALIFICATIONS: Graduateof an accredited school of Occupational Therapy.Minimum of 1-year acute care experience. Prior Home Care experience preferred.

ESSENTIAL FUNCTIONS:

  • Possesses the clinical skills necessary to case manage the Agency's clients
    • Performs complete assessment/evaluation including, but not limited to home safety, pain, nutrition, fall risk, and DME needs
    • Develops/Implements a Plan of Care based on assessment findings and physician order
    • Evaluates client outcomes, communicates progress, and changes in condition to physician
    • Effectively instructs the client about safety, home exercise programs and follow-up appointments. Provides the client with a written home exercise program as well as other indicated written instructions
    • Ensure appropriate utilization of visits and services for care ordered as noted through proper documentation
    • Meets productivity standards
    • Coordinates delivery of care and demonstrates multidisciplinary collaboration
    • Supervises the Home Health Aide per regulation
    • Completes client care documentation in a thorough, reliable analytically sound and efficient manner following Agency guidelines. Records treatment performed, and client's response and progress with treatment
    • Interacts and practice collaboration with all indicated professionals to ensure quality client care, develop and enhance Agency programs, enhance team effectiveness, and increase awareness of Agency programs to the community
    • Adheres to all NCHHA Policies and Procedures
    • Assists with the orientation process of new associates and staff education on an as needed basis
    • Complies with State and Federal regulations
    • Performs other related duties as assigned or requested
  • To produce quality documentation
    • Documentation is to be done in the client's home, during the visit
    • All documentation to be completed within 24 hours
    • Supervisory visits must be documented as per regulation
    • Oasis and 485's completed within the appropriate window of time
    • documented at least weekly on 1st visit of the business week
    • Admission paperwork delivered to Homecare Director by the following day
    • All orders are entered appropriately into the clinical record
    • Manner of documentation completed according to Agency policy and in compliance with regulatory agencies and payer dictates
  • To communicate effectively and professionally
    • Clients know when to expect OT visit and OT adheres to the schedule set up with client on admission
    • Physicians and other appropriate team members are kept informed and such communication can be found in the Communication section of the client's record. Holds Case Conferences as needed
    • Check in with Clinical Director at least every morning to confirm schedule and make/receive any needed changes
    • Leaves voice mail or otherwise communicates with staff about admissions, weekend report, changes in status while on call, etc
    • Addresses problems/concerns in a positive, productive and professional manner. Promotes teamwork and collaboration
  • Practices in accordance with current ethical principles
    • Avoids paternalism and promotes clients right to self determination
    • Maintains client's confidentiality and complies with HIPAA regulations. Any breach of confidentiality will result in discipline up to and including dismissal
    • Delivers care irregardless of race, religion, belief system, nationality, etc
  • Is professional in appearance and behavior
    • Dresses professionally according to Agency policy
    • Wears name tag when in clients home and when in the community representing the Agency
    • Follows Agency policy regarding giving out personal information, seeing patients when off duty, running errands, etc
    • Is punctual for meetings and home visits
    • Has regular attendance. Notifies Homecare Director if unexpectedly absent and discusses the distribution of assigned clients
    • Works as a team member and values the input and wisdom of the team
