**JOB TITLE: SSVF Outreach Case Manager**
**Title**:SSVF Outreach Case Manager**
**Summary**:Conducts and spearheads outreach to the community and other agencies to educate them about the SSVF program and obtain referrals of SSVF-eligible participants who are literally
- and technically “homeless”. Coordinates and provides case management services that are necessary to ensure that the participant(s) access VA and other benefits. Implements case management process to help low-income veteran families to achieve housing stability and become job ready by identifying and overcoming barriers to employment including chemical dependency, physical, mental health, transportation, and housing issues. Assists participants in identifying any additional barriers to employment and/or housing and in obtaining skills needed for stability in life, housing, and employment. Keeps timely and well-documented participant case record notes, making timely and complete entries into the SSVF HMIS system with mínimal data errors. Studies basic SSVF documents, including our proposal, Final Rule, manuals, etc. and participates in required SSVF webinars, conference calls and conferences, retaining updates in SSVF procedures and priorities.

**Salary/Status**:Full Time, Exempt
Maximum annual salary is $48,000 based upon education and experience.
**Manages the following programs and/or services**
- Employment Services - Manages the compliance, program development, and implementation of Employment services. Supervises a minimum of two employees and interns and volunteers.
- Adult Education and Training Workshops
- Manages the compliance, program development, and implementation of program services.
- Youth Services - Manages the compliance, program development, and implementation of support services.
- Data tracking and Quality assurance measures as required by funders
**EDUCATION AND/OR EXPERIENCE**
**Level of Education**:Minimum of bachelor’s degree in Social Work, counseling, or a related field. Substantial related experience in related fields, including community outreach, may be considered in lieu of degree.
**Work Experience**: A minimum of two or more years direct social experience (preferably working with veterans) in social services setting, or other comparable experiences providing eligibility & service assessment services to low-income communities including case management or customer service. Demonstrated knowledge of homeless populations, relevant community resources, and of housing and employment barriers faced by veterans and homeless families. Direct experience in customer service is required. Veteran status and vocational rehabilitation experience are plus factors.
**Substitutions**:Supervision of volunteers and interns can be used for supervision requirement.
**ADDITIONAL QUALIFICATIONS**
**Computer Skills**
To perform this job successfully, an individual should have basic proficiency in database software; Internet software; Spreadsheet software and Word Processing software. Knowledge and experience in database development and/or data management, preferably in a nonprofit setting.
**Language Skills**
Ability to read and write at a professional level: to read, analyze, and interpret and implement general business documents, professional journals, technical procedures, or governmental regulations and write reports, business correspondence, and procedure manuals. Ability to gather data, analyze information and interpret information to build useful and required reports or presentations. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the public.
