**QUALIFICATIONS**
- Minimum age requirement of 18.
- Ability to read, write and follow instructions and document care given.
- Understands Home Health and Hospice philosophy.
- Compliance with accepted professional standards and practices.
- Demonstrates tact, patience and good personal hygiene.
- Excellent observation and communication skills.
- Satisfactory references from employers and/or professional peers.
- Self-directed with the ability to work with little supervision.
- Flexible and cooperative in fulfilling all obligations.

Experience with Homecare Homebase EMR a plus, but will train.

**RESPONSIBILITIES**
- Verifies data of Hospice Care Plans every one-two weeks into electronic database.
- Ensures timely and accurate mailing and/or faxing of physician orders for signature. Retains copies of orders sent and utilizes computer to track receipt. Communicates with physician offices to facilitate order return.
- Ensures that live and complete charts are updated - communicating with Supervisor & staff regarding any and all outstanding paperwork.
- Accurately scans medical records in client charts in a timely manner.
- Sort all incoming paperwork and enter or distribute as appropriate.\
- Tracks missing paperwork.
- Performs clerical chart review.
- Performs discharge functions on the client record.
- Runs clerical reporting in computer system to ensure timeliness of documentations.
- Assists with preparation of IDT meetings.
- Ensures death certificates are signed by physicians in a timely manner.
- Assists with answering phones in a clear, concise, and friendly voice maintaining confidentiality at all times.
