The Pines Home Health and Hospice is a healthcare organization dedicated to providing exceptional care to our patients and their families. We believe in the power of teamwork and collaboration, and we strive to create an environment where every team member feels valued and supported. As a Hospice Aide CNA, you will play a crucial role in providing personal care and promoting comfort for our patients. You will work under the supervision of a registered nurse and be responsible for observing patients, reporting observations, and documenting care provided. This is a time-sensitive role that requires compassion, empathy, and a commitment to delivering high-quality care.

Job Summary:
  • Perform personal care needs and attend to patient requests promptly
  • Ensure patient safety and use equipment properly
  • Provide personal care including baths, oral hygiene, and changing bed linen
  • Assist patients with dressing and undressing
  • Plan and prepare nutritious meals
  • Assist with feeding patients
  • Take and record vital signs
  • Provide proper care and observation of patient's skin
  • Report patient's condition and significant changes to the Case Manager
  • Assist with ambulation and exercise
  • Offer and assist with personal hygiene needs
  • Perform simple procedures as ordered
  • Keep patient's living area clean and orderly
  • Adhere to documentation and care procedures
  • Participate in quality assessment performance improvement activities
  • Respect patient and family/caregiver environment and personal needs
Job Requirements:
  • Successful completion of a state-approved training program
  • Ability to read and follow written instructions
  • Understanding of hospice philosophy
  • Comfortable providing specialized care to the terminally ill
  • Good oral and written communication skills
  • Good organizational skills
