**Statement of Purpose**: This position provides leadership and is responsible for community outreach for promoting programs conducted at the Center for Health & Wellness (CHW). Completes outreach to medical providers, community centers, and other viable referral sources for CHW. Promotes brand awareness.

**Some of the main responsibilities**:

- Develop and implement a marketing and sales plan to maintain effective work relationships with internal and external customers to enhance the use of services provided by CHW.
- Fosters strong business relationships with local community organizations and businesses to increase their knowledge of CHW.
- Networking with local organizations, leaders, and consumers, resulting in establishment of new and lasting referral partnerships and relationships.
- Collaborates with Vice President, Practice Manger, and CHW staff to develop and implement specific outreach, communication strategies, relationship building, and marketing plans to increase provider referrals to CHW programs.
- Identify and pursue outreach strategies in the Southwest Valley and West Valley area to establish new and/or increase provider referrals and community engagement through actives such as vendor opportunities at health fairs, 55+ Communities, community events, and potential new venues to host DSME and DPP/MDPP programs.
- Market, promote and execute Service Provider Agreements to potential users of Revenue Based online platforms and provide ongoing customer support as necessary.
- Leads initiatives to build volume and favorable provider perceptions among internal and external referral streams, by sharing program outcomes and information about Sun Health's charitable mission.
- Develops and implements educational opportunities for physicians, nurses, ancillary staff, and outside medical services. Works with medical staff to identify needed programs and services. May assist with the development of other programs courses, and community medical education presentations.
- Markets medical services and programs for target patient populations. Develops education materials, marketing materials and other promotional materials and makes on-sight contacts to increase awareness of and referrals to target programs/services.
- Acts as a liaison between Sun Health staff and community medical directors, physicians, medical services, administration and referring facilities.
- Demonstrate the Sun Health Vibrant Living culture and provides all customers/audience with an excellent service experience by consistently demonstrating Sun Health behaviors (the 6 Cs) each, and every day.

**Qualifications**:

- Three to four years' experience in marketing, community outreach, sales or related field at a progressively responsible level required.
- High School Diploma or equivalent required. Bachelor's degree preferred.
- Possess current or ability to obtain a Level 1 Fingerprint Clearance Card is required.
- Requires reliable transportation in employee provided vehicle, current Driver's license, and auto insurance.
