**Job Summary**:
The Community Support Services Navigator works one on one with members to identify and reduce barriers to care by identifying needs and linking members to sources of emotional, financial, administrative, or cultural support. In addition, the Community Support Services Navigator helps educate members of the importance of preventive care and the maintenance of the medical home. The Community Support Services Navigator is a health care _representative_ who serves as a link between the member, health care system and community resource/supports.

**Essential Job Functions**:

- Establish effective, trusting working relationships with members.
- Identify and reduce barriers to patient care.
- Link members to community resources, programs and support services
- Help members understand the healthcare system so they can self-navigate, self-advocate and thereby become more satisfied with their health care experience
- Work with healthcare team to coordinate member care and identify need for community resources/ support services
- Seek and maintain current information about available community resources, programs, and services for members.
- Establish and foster professional relationships with healthcare team members and community-based organizations (CBOs)
- Maintain communication with healthcare team members to identify members with or “at risk” for barriers to care.
- Triage Health-related transportation options to maximize independence.
- Maintain working knowledge of all grants and special projects administered by Vivant’s Program Development Department in order to support grant funding requirements
- Fulfill the specific coordination/ navigation responsibilities of all grants or special projects administered by Vivant’s Program Development Department
- Execute all related administrative requirements assigned by department leadership.
- Maintain confidentiality and compliance with Health Insurance Portability and Accountability Act (HIPAA)
- Perform other related duties as required.

**Other Functions**:

- Enforces Company policies and safety procedures.
- Regularly updates job knowledge by participating in educational opportunities.

**Competencies**:

- Three years’ experience in the medical industry.
- Previous “Member Relations” experience in an IPA, health plan, or healthcare setting preferred.
- Excellent oral and written communication skills.
- Exceptional critical thinking skills.
- Active listening skills
- Capacity to solve mid-level problems with mínimal supervision.
- Self-motivated, able to take initiative and think creatively.
- Exhibit professionalism, confidence and empathy while quickly and positively engaging members.
- Relationship building
- Knowing, identifying, and respecting limits of the role as a community support services navigator
- Ability to exercise excellent time management.
- Provide and receive constructive job-related feedback.
- Maintain confidentially and appropriately share information on a need-to-know basis.
- Ability to consistently deliver excellent customer service.
- Outstanding attention to detail and ability to document information accurately.
- Ability to effectively and positively work in a team environment.
- Microsoft Office Suite experience required, including MS Word and MS Excel.
- Reliable transportation.

**Education and Licensure**
- High School Diploma or GED
- CMA
- Valid California Driver’s License

**Travel**

The incumbent may travel up to 25% of the time in the greater Sacramento area.

**Work Environment**

This job operates in a professional office environment. This role routinely uses office equipment such as computers, phones, photocopiers, scanners and filing cabinets.
