As a **Discharge Planner **you will:
- Coordinates discharge planning for a specified patient population in a single hospital.- Follows patients throughout the continuum of care and ensures optimum utilization of resources.- Provides ongoing support for discharge planning through comprehensive assessment of discharge needs to include barriers to compliance with recommended plan of care post discharge.- Provides support and discharge planning for patients who are in need of ongoing medical/psychosocial care following discharge.- In collaboration with other members of the healthcare team, assesses psychosocial needs of the patient and offers suggestions and facilitates the transition to follow-up care options.
**Qualifications**
As a **Discharge Planner **you will have:
- High school diploma or equivalent.- Bachelor’s degree preferred.- CPI- CPR- One year experience in a psychiatric inpatient environment.
