**SUMMARY**

The Integrated Care Manager (ICM) at ESL is responsible for performing advanced indirect and selected direct client care duties in support of the clients’ living skills program in a competent manner under the direction of the Aftercare Program Director. The ICM provides general oversight of the housing environment and reports all environmental needs as it relates to supporting clients, to the operations and Aftercare Program Director. The ICM conducts Case Management duties including but not limited to assessing, planning and facilitating care coordination to meet the client's individual needs, as well as assisting in transportation needs to appointments and providing medication management.

**AGE/CLIENT POPULATION(S) SERVED**

Age of Client Population Served: Adult (18 & older). Population Served: Persons meeting DSM5 criteria for substance use disorders with or without cooccurring psychiatric illness, and psychiatric illness with or without substance use disorders.

**ESSENTIAL DUTIES AND RESPONSIBILITIES**

1. Ensure all clients are woken up by 7:30 a.m. (AM Shift)

2. Facilitate morning meeting/meditation and chores

3. Make sure client’s maintain proper hygiene and dress appropriately

4. Review client schedule daily and specify outside appointments and activities

5. Conduct a weekly inventory of client medications and contact MS Department to ensure timely refills

6. Observe clients taking daily medications as prescribed adhering to medications times

7. Communicate with the MS Department and Directors of medication incident reports and med changes needed.

8. Facilitate client transportation to CCI and outside appointments and activities in a timely manner

9. Conduct meal planning sessions and ensuring clients have listed all necessary ingredients for their meals and create the grocery list

10. Manage weekly household expenses providing receipts and reporting remaining funds

11. Work with each client individually to assist them in attaining client treatment goals

12. Counsel clients as needed for behavioral issues and schedule emergency psychiatric and/or individual therapy appointments when necessary

13. Maintain and update Client Task Manager spreadsheet daily

14. Retrieve client packages from CCI office

15. Attend weekly ESL Staff Meeting

16. Update Master Schedule Board and Client Information Station with schedule changes, weekly menu summary and chore list

17. Implement weekly UA test schedule and confirm

18. Oversee preparation for new clients in housing (make bed, notify existing clients of new addition to their room, give Welcome flyer, etc.)

19. Ensure clients are attending three outside meetings a week

20. Collect and review client Meeting Attendance Sheets, Phase-up and Pass Requests

21. Schedule daily store-runs for clients and manage cash and/or credit cards dispensed (as needed)

22. Conduct Pass down with ICM Staff using designated passdown template

**MINIMUM JOB REQUIREMENTS**
- Education, Training, and Experience Required:_
- High school diploma or equivalent and one (1) year of experience in behavioral health care, preferred. - Attainment of at least 18 years of age
- A Health Screening with negative TB Test Results Current CPR/ First Aid certification.
- D.L. or Government I.D.
- Should be experienced house manager or Mental Health Technician, preferably in the field of chemical dependency and/or behavioral health.
- Required Knowledge, Skills, and Abilities:_

1. Knowledge of industry standards as related to the position of The Integrated Care Manager.

2. Basic knowledge of PC software programs, particularly Microsoft Word and Excel is preferred.

3. Written and oral communication skills; must be able to concentrate on work amid distractions such as noise, conversations and foot traffic

4. Must maintain self-control in volatile or hostile situations such as when verbally or physically confronted in an aggressive manner

5. Must be able to work independently with little supervision in an unstructured environment with multiple demands

6. Must possess a reading and writing level that provides for clear expression and/or understanding of assignments related to job description.

7. Ability to represent the program well in interactions both inside and outside the facility
