Job Overview

We are seeking a skilled and experienced Care Manager to join our leadership team at Assisting Hands Serving Carlsbad and Oceanside. The successful candidate will play a pivotal role in overseeing the delivery of care solutions and services to our clients, working directly with our owners to ensure seamless coordination and exceptional care.

Responsibilities
  • Conduct comprehensive assessments of client needs and develop individualized care plans in collaboration with clients, their families, and other healthcare professionals.
  • Coordinate and schedule care services based on client preferences, availability of caregivers, and specific care requirements.
  • Recruit, train, supervise, and evaluate caregivers to ensure they deliver exceptional care in accordance with company policies, procedures, and standards.
  • Conduct regular home visits to assess client satisfaction, monitor the effectiveness of care plans, and address any concerns or issues that may arise.
  • Serve as a point of contact for clients and their families, providing support, guidance, and advocacy as needed.
  • Collaborate with healthcare providers, including physicians, nurses, therapists, and social workers, to coordinate and effectively integrate care services.
  • Maintain accurate and up-to-date documentation of client assessments, care plans, and service delivery in accordance with regulatory requirements and company standards.
Qualifications
  • Minimum of 3 years' experience as an LVN and/or experienced homecare manager.
  • Skilled problem solver with a can-do attitude.
  • Strong interpersonal, communication, and leadership skills with the ability to collaborate effectively with clients, caregivers, and office team members.
  • Proficiency in computer skills, including Microsoft Office applications. Experience with care management and scheduling software preferred.
  • Compassionate, empathetic, and client-centered approach to care management.
  • Valid driver's license and reliable transportation.
Compensation

$45.00 per hour
