As the **Nurse Practitioner**, you will have the opportunity to collaborate with the Clinical Team to contribute to that mission. These passionate team members will depend on you to provide comprehensive care to hospice patients, and to be a reliable go-to problem solver.

**On an average day, you will**
- Provide oversight of clinical and Nurse Practitioner services in coordination with the Medical Director
- Assist with on-call needs when the Medical Director is unavailable
- Provide oversight on initial and ongoing assessment of the impact of the terminal diagnosis on daily living, and the patient's physical, functional, psycho-social, and environmental needs
- Partner with an interdisciplinary team to implement and manage adequate, individualized plans of care, determining scope, priority, and frequency of services based on acuity and patient/family needs
- And perform additional, related duties as needed

**Qualifications/Desired Skills**
- Are passionate about providing the best possible care to patients and their families

- Have at least 1 year of Nurse Practitioner work experience, preferably in hospice
- Have an active Advanced Practice Registered Nurse (APRN), CNP, or other NP license
- Are able to travel within the service area of Brighton (current driver's license, personal vehicle, and auto insurance)
- Are compassionate in caring for the sick and terminally ill
