**Scope of Job**:
Plans and delivers rehabilitation services to children and adolescents.

**Job Duties**:

- Establishes a professional, caring relationship with all people being supported by the team, and their parent(s)/caregivers(s), if residing with a parent/caregiver. Has awareness of and is sensitive to each person's cultural, racial, and ethnic background; facilitates voluntary participation in activities and events related to the person's background.
- Completes assessments and Individual Rehabilitation Plans (IRPs) and assists each person with meeting the goals and objectives of the plan; integrates existing plans from other service providers to ensure coordination of services; provides each person with life skills training appropriate to the age and capability of the child/adolescent; designs services to meet individual needs for guidance, learning, and personal development.
- Provides services in the natural environment such as the home, school, job setting, or elsewhere in the community. Makes use of natural supports whenever possible. Links each person to appropriate community resources such as education, recreation, employment services, mental and physical health services, entitlements etc.
- Works in partnership with the family, caregiver, and/or other service providers, ensuring clear and consistent communication and coordination of services; meets regularly with the family or caregiver; teaches the family or caregiver how to follow through with the IRP when staff is not available.
- Completes all records and documentation requirements on the appropriate forms within the established timeframes, meeting professional standards (e.g. the IRP, daily contact notes, monthly progress reports, health status reports, and other forms required by the agency). Strives to meet standard for face-to-face time. Documents interventions via the Electronic Health Record (EHR). Maintains EHR, including uploading proper documentation according to policy.
- Serves as a liaison with parents/caregivers, schools, juvenile court, and other social and community services. Provides indirect services such as service coordination, telephone contacts and follow-ups. Maintains contact with treatment providers to ensure child, adolescent remains active with treatment. Has knowledge of and abides by all laws pertaining to children and adolescents, including those related to confidentiality.
- Attends meetings as required. Trains new staff and interns on the job duties of the team's HSW staff. Focuses on meeting all COMAR, CARF, HIPAA and other regulatory requirements. Attends certification/recertification training as scheduled. Completes all other training as scheduled/assigned.
- Performs other tasks related to the overall scope of the job as assigned.

**Qualifications for this Position**:

- High School Diploma or equivalent and 1 year experience working in a supervised mental health setting required. Thirty college credits, AA Degree, or BA/BS degree in a health related field preferred.
- Associate's, Bachelor's, or Masters degree allows for annual education stipend
- Strong interpersonal skills
- Good documentation skills

Pay: $19.00 per hour

Expected hours: 40 per week
