**General Description**

The Community Health Coordinator will lead community health activities and efforts to improve health and promote wellbeing among residents through health promotion programs, needs assessments, strategic planning, and grant-funded opportunities. Some supervisory capacity including assignment of staff, staff development and measurement of outcomes, and oversight of the student internship program. This position reports to the Director of Health and will support in meeting Health District program objectives.

**Essential Job Functions**
- Actively participates in the development, implementation and completion of Health District program goals as directed by the Director of Health.
- Assists in the administration of the operations of the District as directed by the Director of Health.
- Supports community health work of the Health District: assists in identifying community needs; leads development, implementation, and tracking of programs and initiatives to improve the health and wellbeing of residents; and builds relationships with community partners and identifies opportunities for engagement.
- Provides direction to and supervises community health staff.
- Participates in grant writing opportunities and grant program implementation.
- Supports the Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP) process: tracks CHIP progress and reporting; organize meetings; conduct assessments as needed.
- Coordinates the efforts of community health: schedules meetings; prepares agendas and keeps records; conducts surveys, analyzes and reports on results; prepares reports and presentations; manages outgoing communication including flyers, website and social media.
- Analyzes data to evaluate and support program success and modify programs for maximum effectiveness.
- Identifies community health-related training needs and opportunities.
- Oversees Health District student internship program.
- Serves as a liaison to community and State agencies and coordinates with other services and departments as needed.
- Maintains a responsive and professional public image.
- Maintains positive employee relations.
- Other duties as assigned.

**Additional Duties**
- May be required to attend night meetings.
- May be required to work evenings and weekends.
- Participates in professional education and training.

**Minimum Qualifications**
- Master’s degree from an accredited college or university in Public Health, Public Administration, Health Policy, Healthcare Management, or a closely related field from an accredited college or university with a minimum of 2 years of experience in a community health or public health environment.
- A minimum of 2 years of successful community engagement related work experience with community partnerships, community members, community-based organizations and/or governmental agencies preferred.
- Hold a current valid motor vehicle operator’s license.
- Have a motor vehicle that can be utilized on District business.

**Knowledge, Abilities, and Skills, Advisement and Programming**
- General knowledge of public health, local public health practice, public health programming and evaluation methods, community health assessment methods, policy development, behavioral science, and community organizing practices.
- General knowledge of public health systems and interaction of health system; familiarity with the 10 Essential Services of Public Health.
- Knowledge of infectious and chronic disease risk factors and processes.
- Knowledge of the influence of socioeconomic status, culture, and race/ethnicity on health practices and health outcomes.
- Commitment to Diversity, Equity and Inclusion and reducing health disparities.
- Considerable ability to communicate ideas effectively both orally and in writing.
- Demonstrated organizational skills with the ability to organize and facilitate groups and meetings.
- Analytic and assessment skills to address health status of populations and determinants of health.
- Demonstrated ability to work in a team-oriented, highly iterative, and collaborative environment.
- Demonstrated ability to foster cooperation, communication and consensus among groups and teams as well as lead, influence, coach, and guide others to help them achieve desired results.
- Computer proficiency required in the areas of Microsoft Word, Outlook, PowerPoint, Publisher, with proficiency in internet/web and social media.
- Proficient in data analysis and creation of action plans.
- Working knowledge of survey development and survey techniques.
