For community-based older adults, we offer adult day health care and home care services to help them remain in the community with the care and support they need. Both our Manhattan and Westchester facilities are accredited by the Joint Commission, and last year, our Manhattan facility earned the highest possible assessment by U.S. News & World Report for its rehabilitation program. Our Manhattan skilled nursing facility is the only one in NYC to staff full-time Mount Sinai physicians and nurse practitioners.

The Nursing Department on the Manhattan campus is seeking a full time Clinical Care Manager for the night shift to join our team This position will sit in the Upper West side of Manhattan. This role is entitled to $ 5000 sign on bonus.

Job Summary:
The Clinical Care manager is responsible for assuring approved quality standards of care and compliance with established policies and procedures in the provision of resident/client care.

Duties and responsibilities:

- Directs and supervises the delivery of nursing care within an assigned unit.
- Delegates the responsibility for direct resident care to the appropriate nursing staff member.
- Monitors care to assure care is provided as directed by the resident plan of care.
- Effectively communicates resident and unit needs to Nursing Administration.
- Utilizes a process of data collection, assessment and care planning to meet the needs of residents.
- Facilitates communication and problem solving with other disciplines to develop a plan of care which will address these goals.
- Responsible for assuring the Resident Care Plan is revised as necessary to reflect the current needs of the resident.
- Participates with Nursing Administration in meeting clinical and budget goals.
- Evaluates staff assigned to her/his assigned unit to determine staff competencies and/or the need for education. Assists, as appropriate in the education of staff. Participates in the counseling and discipline of staff.
- Maintains personal competencies to assure the ability to provide resident care and evaluate the competencies of staff assigned to his/her unit and to serve as a role model for unit staff.
- Communicates resident needs, strengths and conditions to appropriate staff members through unit conferences and in written format.
