Social Services Coordinator

CIS Management is looking for a Social Services Coordinator Role in our Elizabeth, NJ Location

**Responsibilities**:

- provide general assistance and advocacy related related to supportive and social services & refer and link residents to supportive services available in and provided by the community
- Develop and maintain linkages with community resources
- educate residents to services available on site and the in community & refer and link residents to the support services that are available
- create new services or increase the availability of existing services to meet resident needs
- work with families and resident when planning for relocation to a nursing home or other long term care facility
- address the social and recreational needs of the senior community
- promote a positive social climate for the residents
- document contact with residents, providers and families. Keep resident files current

Service Coordinator Qualifications
- Bachelor in Social Work, Psychology or Gerontology or Counseling or relevant work experience
- Demonstrated ability to provide effective communications with individuals with disabilities and ensure meaningful language access for persons with limited English proficiency
- Knowledge of federal nondiscrimination laws; and legal liability issues related to providing voluntary Service Coordination
- Two to three years' experience in social service delivery for low-income youth, adults, seniors citizens and/or people with disabilities
- Demonstrated working knowledge of voluntary supportive services and other resources for youth, adults, senior citizens, and/or non-elderly people with disabilities available in the local area
- Demonstrated ability to advocate, organize, problem-solve, and provide results for low income individuals and families, the elderly, and/or individuals with disabilities; and (8) Ability to deliver eligible activities in compliance with nondiscrimination requirements and have experience working with underserved communities.

Pay: From $19.00 per hour

Schedule:

- 8 hour shift
