**Location**:Riverside, CA
**DAYS** - Monday
- Friday 8:30AM-5:30PM

**Responsibilities**:

- Identify, plan, and coordinate care for high-risk patients (frail, chronic disease, catastrophic injury) in an ambulatory setting.
- Ensure continuity of care across the continuum.
- Promote and evaluate quality and cost-effective outcomes.

**Qualifications**:

- **Education**: High School Diploma or GED required. Graduate of an accredited LVN Program. A.A., Diploma preferred.
- **Experience**: 1-2 years related experience and/or training; or equivalent combination of education and experience.
- **Licenses**: Current California LVN license.

**Skills**:

- Ability to read and interpret documents, write routine reports, and speak effectively before groups.

**Job Type**: Contract

Pay: $42.00 - $44.00 per hour

Expected hours: 40 per week

Standard shift:

- Day shift

Supplemental schedule:

- Overtime

Weekly schedule:

- 5x8
- Monday to Friday

Work Location: On the road
