About the Role

We are seeking a skilled Registered Nurse to join our team as a Case Manager in our Inland region. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to provide comprehensive care and support.

Key Responsibilities
  • Coordinate patient care, ensuring continuous assessment and implementation of individualized plans
  • Provide education and training to patients, families, and caregivers on palliative care and symptom management
  • Collaborate with healthcare professionals to identify and utilize resources, referrals, and services
  • Document patient information and maintain accurate records
  • Participate in patient care meetings, bereavement support, and on-call rotations
Requirements
  • Current CA RN license
  • BSN or LVN with over a year of experience
  • Ability to work in a fast-paced environment and prioritize tasks effectively
  • Strong communication and interpersonal skills
