As a shift care LVN, you will be responsible for delivering exceptional patient care and support to individuals in need of assistance with daily living activities and medical needs.
Key Responsibilities- Assessment and Documentation: Conduct thorough assessments of patients' conditions during every visit, documenting observations and charting vital signs.
- Medical Administration: Administer medications and treatments as ordered by healthcare professionals.
- G-Tube Care: Provide specialized care for patients with G-Tubes, ensuring their needs are met and their comfort is maintained.
- Communication and Feedback: Offer timely and appropriate feedback to RN Case Managers and other healthcare professionals regarding patient progress and concerns.
Care Essentials Home Health offers a variety of shift options to accommodate your schedule and preferences. You will be required to work the following shifts:
- Thursday and Friday: 8am-8pm (12 hours)
- Saturdays: 5am-12pm (7 hours)
- Sundays: 9am-9pm (12 hours)
- Wednesday and two other NOC shifts during the week: 9pm-9am (12 hours)
To be successful in this role, you must have:
- A current LVN license in good standing
- Experience in home health care or a related field
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
