**Position Purpose**:
The Patient Navigator will work closely with healthcare teams, resource providers and key stakeholders including the City of Hope (COH) to improve coordination of patient care to overcome barriers to cancer screening. The Navigator will assist clients and family members by identifying their needs, linking them to resources, and directing them through the appropriate administrative system and care continuum. The staff will also be expected to actively participate in educational and group programs, and outreach to patients, families, with City of Hope departments and the community.

**Principal Responsibilities**:

- Reach out to clients to provide and promote awareness on the importance of cancer screening. Enters data on encounters in patient navigator database of outreach efforts.
- Track all positive cancer screening tests and entered into a registry managed by the QI Department.
- Function as the central care coordinator for the patient and will call the patient after a provider referral is placed.
- Track completion of follow-up consultations and diagnostic tests and coordinate with case managers for community referrals to address social determinants of health (e.g., transportation, housing).
- Performs data entry on Excel spreadsheet to track treatment progress
- specialty consultations, diagnostics and follow up visits.
- Perform an initial assessment of barriers to care using a barrier checklist and emphasize the importance of follow-up with the PCP and healthcare team.
- Coordinate with clinical care team to address any medical barriers identified.
- Coordinate with medical care team for referrals to other specialty departments such as dental care, SUD, Optometry, Chiropractic care, case management and nutritional support.
- Documents services provided, referrals in the EHR system.
- Coordinate transportation for screening or medical appointments, if needed. May include accompaniment.
- Connect clients for additional emotional support and assist clients with scheduling cancer treatment, if necessary.
- Connect and refer patients to Behavioral Health for additional emotional support.
- Assist clients from diverse cultural backgrounds in understanding the Health System.
- Maintain communication with clients and healthcare team and serve as a liaison between clients and medical staff.
- Represents the clinic in all matters dealing with the delivery of health care services and works closely with the provider and healthcare team.
- Attends general staff meetings and community outreach screening events.
- Assist in training for patients on how to utilize virtual platforms.
- Participate in skill building workshops regarding cancer screening, diagnostics, and treatment.
- Other relevant tasks as assigned.

**Requirements**:

- Experience working with clients and staff from diverse socio-economic, ethnic and cultural backgrounds.
- Ability to communicate effectively both orally and in writing.
- Experience working in a healthcare setting.
- Ability and desire to work evenings and weekends (may be required).
- Posses a current California Driver License.
- Ability to travel to different work sites.- LVN or Medical Assistant experience preferred.
- Bilingual (English, Spanish) preferred.
