:* This position would communicate with staff, physicians, referral sources and prospective clients regarding hospice services and appropriate admission criteria. Handle initial physical and emotional assessment of the patient/family upon admission and reviews the hospice concept with them. Develop a detailed, written POC for each patient admitted in conjunction with the IDT, patient, family and attending physician. *Qualifications:*
- Graduate of an accredited school of nursing with current Texas license as a Registered Nurse.
- Current certification in basic life support.
- Excellent organizational skills to handle a variety of tasks simultaneously.
- Effective interpersonal skills.
- Excellent communication skills, both oral and written.
- Ability to accommodate multi-faith beliefs and backgrounds of patients/families.
- Ability to function as part of interdisciplinary team.
- Ability to travel to multi-work locations during workday.
- Must have reliable, insured vehicle and valid Texas Driver’s license. Job Type: Part Time with Various Pay Rates Based on the type of visit conducted: Initial Admission, Routine Visits, Hourly (IDT Meeting), and Comprehensive Assessments Experience:
- Hospice: 2 years (required) License:
- RN Pay: $50.00 - $75.00 per hour Expected hours: No less than 20 per week Medical specialties:
- Hospice & Palliative Medicine Physical setting:

- Outpatient Standard shift:
- Day shift
- Evening shift Supplemental schedule:
- On call Weekly schedule:
- Monday to Friday
- Weekends as needed License/Certification:
